Here is a comparison of many of the shareware registration sites:

I use BMTMicro mostly because it was the easiest to set up with.

Cafe press sell's CD's which aren't really practical for an internet market... Unless...

A. you are selling to dialup users.
B. you are selling a large application.
C. you are selling a CD bootable application but your users are not capable of burning ISO's

however it would be practical for a local market (IE: you buy the CD's and redistribute them yourself) but then it would be more cost effective to buy a burner and some sort of CD labelling device (I use the CD stomper pro, however the quality and cost aren't really worthy of the term "pro", but it is good for internal labelling)

Hmm... well if you get a decent printer by todays standards, you should be able to make rather nicely labeled CDs by buying some of the pretty nice kits available in FutureShop or Office Depot type stores. I think maybe even Kinkos will be able to offer you a decent price on these lables aswell.

Of course this is all only for do-it-yourselfer type production. And it may be harder to do things this way if you have such a good game that you are planning on having the next Doom-type milestone.
